USA THERAPY DOGS SPRING Class
Start Date: Classes will start Tuesday, February 23, 2016

Hadley Acres Obedient and Care

13 weeks total – we will meet 12 consecutive classes for instruction, last week is graduation
Time: First Class: 6:30 to 8:00
Membership applications can be found on the USA TD website www.usatherapydogs.org
Go to the FORMS tab; go to GETTING STARTED, and AGREE TO GUIDELINES
Print and fill out the following forms:
1. Canine Information form
2. Vet Authorization/Vaccination form (must be signed by your vet) must use USA TD form
3. Membership application form
In addition to the forms please send in a copy of your
1. Driver’s License front and back
2. Current County Dog License
3. Record of having completed the AKC C.G.C
Send the completed forms and payment (see below for cost) to:

Someone will contact the applicants for a prescreening
Preescreening will be done at Hadley Acres Obedient and Care 9:30 am to Noon Ferbruary 13
We request that all paper work is submitted by February 5, 2016 along with payment

Cost:
$195 for one handler and one dog
$245 for two handles and one dog
